You can submit your requests online through HealthMark’s Request Manager.

  3. Download your records straight to your computer

Status updates are sent via email, and you will be notified once your records are ready for download.

Need your FMLA or disability forms completed?

Image of steps to submit to receive your FMLA or disability forms. Step 1 - Scan and Submit Request. Step 2 - Healthmark completes forms (takes 3 days). Step 3 - HealthMark sends for doctor signature (takes 1-2 days). Step 4 - Physician returns forms (takes 1 day). Step 5 - Invoice and forms are ready (takes 1 day). Step 6 - Make payment and access forms.

The total process time of requests is approximately seven (7) business days.

Form Fees:

  • First form fee – $45
  • Two (2) or more forms (submitted together) – $45
  • Form updates – $25
    • Forms that exceed four (4) pages may incur additional fees.
    • You will be notified of all fees prior to the release of forms, and forms are released only after payment.
    • Initial statuses are available 48 hours after submission
